Thank you for your interest in the Kitchener-Waterloo Symphony. Please fill out this Donation
request form in order to be considered for a charitable donation of concert tickets.
The following conditions must apply to all applicants:
- Organization must be a registered charitable organization
- Donation request must be submitted at least 4 weeks prior to the date of the event
- Organizations are invited to apply once each 12 month period
